Call 111 when it's less urgent than 999 

You can use the NHS 111 service if you urgently need medical help or advice but it's not a life-threatening situation


Appointment cancellations: please use the link on Appointments Screen 'Cancel your Appointment online' .  If you send a message via the comments box we may not receive it in time


A doctor's appointment is not usually required for sick notes, repeat medication requests, insurance forms.  If in doubt please contact the doctor's secretary before booking an appointment

Carer Questionnaire

The local CCGs are working together with Bradford Council to understand the experiences of carers across Bradford District and Craven. They want to know what’s most important to carers so that we can offer the right support.


A carer is anyone who looks after a family member, partner or friend who needs help and cannot cope without their support. The person they care for may need help because of illness, frailty, disability, a mental health problem or an addiction.

We have launched a Carers Survey which we would encourage all carers in the Bradford District and Craven to complete as part of our wider engagement activity. The survey can be completed using this link Carers Questionnaire

The deadline for completion of the survey is 23rd July.

The online survey is just one way that you can share your views.


If any carers would prefer to complete a paper version of the survey it can be printed off from the link below.  This may be returned by freepost – please see below for details:
